Dear Parent or Guardian: 
 
Immunization requirements for incoming 7th graders- California immunization law requires that all incoming 7th grade students provide proof of a current Tdap (given after 7th birthday) and 2 Varicella (Chicken pox) vaccinations.
 
Students who do not meet these requirements will be excluded from attendance as specified in the California Health and Safety Code section 120335. 
 
Please consider the following: 
      1. If your child has already received all of these immunizations, bring us the immunization record so that we can update our files. Your child’s record must include a date for the immunizations listed above and the doctor’s/clinic’s name. 
      2. If your child has not already received all of the immunizations required, bring this form along with your child’s immunization record to your doctor to get the missing immunization(s). Bring us your child’s updated immunization record so we can update our files.
      3. If any of these immunizations were not given to your child because of a medical reason, please contact your child’s physician to initiate a medical exemption through the CAIR-ME process. For more information on medical exemptions, please visit: https://cair.cdph.ca.gov/exemptions/home
